| I have the honor to return the form of
oath properly filled up and attested which is required to be sworn to
and subscribed when accepting a commission. |
| I am thirty one years ten months of age,
reside in Pulaski County Georgia, was appointed Second Lieutenant
Corps of Engineers to rank from Nineteenth day of October one thousand
eight hundred and sixty four (1864) and was born in Georgia. |
